Excel Manager’s Player & Coach Registration Process
2010-2011 Season
The following is the process to get your team registered
for the 2010-2011 Season:
Significant Dates:
June (age dependent)-
Last formal CCV tryout
June/July (age dependent)
– Team rosters posted on CCV web site
June/June
- Manager Meeting/Final registration
August 16
– first week of practice
From the time the “age group” has been posted or announced, the manager should begin the following process:
Player and Coach Registration will be done on-line, but with assistance from the team manager and through Fran Demos (club registrar).

to review your team registration. At your meeting bring the following:
Each player needs to have registered on-line at www.ccvstars.com. The manager should do the following:
Confirm that all players chosen have registered and paid on-line. If not manager should collect manual registration form and money and turn into Fran.
Registration fee; $180 (u7 '04); $200 (u8-17 ‘03/‘94)
If partial check because of a scholarship request then
scholarship form
should be sent to Shami Bowles at

(for Harry Demos) for approval before player is approved to play and officially rostered.
Collect the player's birth certificate (if player did not play in Excel last year)
AYSA Membership Form
(requires notary for medical release) – may use Excel Manager Serina Shaw for this service (serinashaw@aol.com).
AYSA Code of Conduct
(to be signed by both player and parent)
One inch photo (head shot) for player pass.
Each Head or Assistant Coach needs to have the following forms completed:

(only if you are have not already done so in the past two years)
CCV Background Check
(only if you are have not already done so in the past two years)
AYSA Volunteer Registration & Disclosure Form
AYSA Code of Conduct Form
(to be signed by coach for each team coaching)
One inch photo (head shot) for coach pass.
Electronic photo (head shot) in jpg. Format (sent to Shami)
Each Manager needs to have the following forms turned in:

(if they are to have a sideline pass only)

NOTES:
All one inch photos should stay in the possession of the manager to be used for the player/coach/manager passes.
You should make a copy of all forms from the registration process above. One copy stays with STARS and one will stay with you.
Please make sure that every player on your team receives a copy of the
Excel Player & Family Acceptance Agreement
. No signatures are needed, but all players and parents should review.
AYSA League (State or Open) TEAM Registration will be completed by Fran and Harry in mid August. Harry will consult with your coach about making sure that your team is in the correct division. All league team registration and referee fees will be paid by STARS. STAY AWAY FROM ANY LEAGUE REGISTRATION UNTIL NOTICE FROM HARRY.
Uniforms – handled by the team manager directly with our supplier Eurosport. Go to the STARS web site, in the SOCCER section, then STORE and then Excel game attire. Practice shorts and socks can be attained the same way as game uniforms. Practice t-shirts can be purchased directly from STARS via on-line order form.
